Referrals come from the CMHA-WECB CTO coordinator at Hotel-Dieu Grace Healthcare. Assists individuals by providing treatment, care, and supervision in the community instead of a longer stay in hospital or an alternative to being hospitalized. The Community Treatment Order plan identifies what the person and their supports must do to help them stay well and out of hospital. This includes intensive case management supports, assertive outreach, assessments, information and education, support in coordinating services and links to resources, service plans based on individual needs, supportive counselling, crisis-planning, crisis intervention, crisis prevention, assistance with symptom and behaviour management, medication support, skill-development, family support and education, advocacy, and professional/public education.
