Dual Diagnosis Consultation, Outreach, and Stabilization Service
Description
An outreach stabilization service for those individuals, 16 years of age or older, with a pre-existing Developmental Disability indicated by an IQ of 70 or less, and also have a Psychiatric Disorder. A two to three (2-3) hour assessment will take place to determine if the individual is eligible for Dual Diagnosis Consultation services. The assessment will be completed by a registered nurse and will take place at HDGH or potentially in the individual’s home. Based on the information collected, an intervention plan will be created to monitor and stabilize the individual’s symptoms. A meeting will then be set up with the individual, their family or caregiver, the nurse, and the program’s Psychiatrist. The client’s healthcare team, along with their primary healthcare provider, and community organizations will work together to create a personalized treatment plan.

Due to the specialization of the program, referrals to the Dual Diagnosis Program may ONLY be received from primary care providers or psychiatrists via hospital EMR or referral form
