The Transitional Youth Psychiatry Clinic provides a psychiatric assessment with ongoing support for emerging youth ages 16-24 as they transition from the child and youth mental health sector and into the adult mental health system. This program will help to empower, assess barriers, and support system navigation to ensure an appropriate and ongoing connection as the youth transitions into their adult lives. Priority is given to those emerging youth with high needs, those that require multi-disciplinary support beyond what their family doctors can provide. They will be followed by the clinic physician until they are able to be connected to an adult psychiatrist for ongoing care.
